Medetomidine is replacing xylazine as a fentanyl adulterant, and most standard immunoassay panels were never built to catch it. Find out where your current panel has a gap, before a patient does.
Medetomidine isn’t an addition to the fentanyl supply. In the health systems tracking it, it’s a replacement, and it comes with a withdrawal syndrome that doesn’t respond to the same treatments as fentanyl or xylazine withdrawal. Clinicians who don’t know to look for it are managing a condition they haven’t been trained to recognize.
Standard FDA-approved immunoassay panels don’t cover xylazine, medetomidine, or nitazenes. A clean 10-panel result doesn’t rule out what’s actually circulating; it just means the panel wasn’t built to see it.
